Our care plans are custom-made processes that include correct identification of existing needs, as well as recognizing potential needs or risks. Our care plans also provide an elaborate means of communication among the care givers, their patients, and other healthcare providers to achieve optimal health care outcomes.

  • Professional risk assessment and situation/condition evaluation
Risk assessment is a straightforward and structured method of ensuring the risks to the health, safety and wellbeing of our clients (and others) are suitably eliminated, reduced or controlled

The main purpose of risk assessments are:
To identify health and treatment hazards and evaluate the risks presented within the homestead
To evaluate the effectiveness and suitability of existing care measures
To ensure additional care (including procedural) are implemented wherever the remaining risk is considered to be anything other than low.
To prioritise further resources if needed to ensure the above.
  • Patient care policies and procedures
Healthcare policies are a set of general plans of action used to guide desired outcomes and are fundamental guidelines that help make decisions. The purpose of healthcare policy and procedures is to accurately communicate to employees to action the desired outcomes of the organization.
